Pilots and controllers sent NASA descriptions of their own errors and near misses, voluntarily and in confidence. This is the seventh quarterly summary, covering October through December 1977, when the office received 1,193 reports. Battelle's Columbus Division staffed the intake. The reports are grouped under winter operations, judgment and decision making, and charts and flight information.

An accident investigation starts from wreckage. These pages start from the narrow miss, with no casualty list attached. G. Courtney Chapman contributes a longer study of how pilots at uncontrolled airports, where no tower sequences the traffic, come to be on conflicting paths. The alert bulletins at the back are the system's second output: hazards in navigation aids, airport lighting and military-civil coordination, passed to the agencies responsible for them.
