Fatal Naphtha Release and Fire at BP-Husky Toledo Refinery (CSB report 2022-01-I-OH)
The U.S. Chemical Safety Board's 169-page investigation report, published June 2024, on the fatal naphtha release and fire of September 20, 2022 at the BP-Husky refinery in Oregon, Ohio.

Four safety issues are printed on the cover of the Chemical Safety Board's report on the fatal naphtha release and fire at the BP-Husky refinery in Oregon, Ohio: liquid overflow prevention, abnormal situation management, alarm flood, and learning from incidents. The fire was on September 20, 2022. The report, number 2022-01-I-OH, was published in June 2024 and runs 169 pages.
Overflow prevention is the engineering barrier meant to stop a vessel from filling past its limit. An alarm flood arrives when more warnings sound than an operator can read, and abnormal situation management covers how operators respond when a process leaves its normal range. The fourth, learning from incidents, concerns what a company does with earlier accidents.
The board describes itself in the report as an independent federal agency, and its stated duty is to report in writing the facts, conditions and circumstances of an accidental chemical release. Twenty-one months passed between the fire and that written report.
