NTSB Safety Study: U.S. Air Carriers, 1978-1990
U.S. National Transportation Safety Board review of major aviation accidents involving U.S. air carriers between 1978 and 1990. The study analyzes flightcrew-involved accidents across the twelve-year period and identifies recurring factors and safety lessons.

The National Transportation Safety Board examined the pattern of major accidents involving U.S. air carriers from 1978 through 1990. This 106-page report draws on the detailed investigations of individual accidents to present the patterns that emerge across incidents. The study identifies the role of flightcrew factors in major incidents and consolidates findings across the twelve-year period. Findings from safety studies of this scope inform changes to training, procedures, and aircraft design standards. The decade covered—late 1970s through 1980s—saw significant changes in cockpit automation and crew resource management protocols, driven partly by systematic review of accident sequences.
