National Aeronautics and Space Administration (NASA) Selected Apollo & Shuttle Lessons Learned 2014
Presentation slides from NASA's NESC Academy videos on Apollo program and Space Shuttle lessons learned. 61 pages. FOIA response.

The NASA Engineering and Safety Center (NESC) Academy publishes instructional videos on space program failure modes and recovery strategies. This document contains slides from their Apollo and Space Shuttle series: the engineering decisions, failure cases, and post-incident analysis. The slides are restricted on NASA's website and were obtained through FOIA. It is institutional knowledge: what NASA teaches its own engineers about why things went wrong and what lessons were drawn. The slides represent decades of aerospace engineering experience compressed into learning modules, the technical culture of failure analysis that became essential to human spaceflight after each major incident in the Apollo and Shuttle programs.
