General Electric's Daytona Beach operation prepared this compilation for the National Aeronautics and Space Administration under a contract identified on the cover as a safety task. It runs 194 pages and collects accident and incident summaries from the manned space programs between 1963 and 1969. The scanned cover carries a case file copy stamp.

The cover notice states that the report was prepared as an account of government-sponsored work, and it disclaims any warranty on the accuracy, completeness or usefulness of the information in it. The 1970 to 1971 volume, prepared by Cranston Research of Alexandria, Virginia under a separate contract and running 139 pages, is already in this archive. The two reports have different authors and different contractors, and between them they cover 1963 to 1971 in one continuous series of summaries.
